Each health care facility (HCF) must maintain a medical record for each patient for six years from the date of discharge that documents the patient’s needs and assessments and services rendered. Records must be maintained as necessary to verify the information and reports required by statute or regulation. The HCF must maintain an adequate medical record-keeping system and take appropriate measures to protect the records against theft, loss, destruction and unauthorized use. The HCF must have policies and procedures to ensure the confidentiality of patient medical records, and may only disclose such records as provided by state and federal laws and regulations.
