“Interdisciplinary records of care; confidentiality under the public health law”
Hospice agencies must keep up-to-date, interdisciplinary record of care being given and patient and family status. Patient records must be retained for a period of 5 years after termination of hospice services. Patient records are confidential and may only be released with patient consent, a court order, or a state or federal agency that requires submission of aggregate statistical data.
