“Patient records under the public health law”
A home medical equipment provider must maintain a record for each patient and retain the record for 5 years following termination of services. The record must contain:
- Any physician’s order if the equipment was ordered by a physician;
- Signed and dated delivery slips verifying delivery;
- Notes reflecting all services and maintenance performed;
- The date on which rental equipment was retrieved; and
- Other appropriate information.
If a patient transfers to another home medical equipment provider, a copy of his record must be provided to the other home medical equipment provider upon request.
