“Medical records of physicians relocating or terminating practice; retention, disposition, time limitations under the Department of Health regulations”
A physician must maintain written medical records for a period of 5 years from the last patient contact. Medical records are confidential and may not be disclosed excepted as provided by law. When a physician terminates his practice or relocates and is no longer available to patients, patients must be notified of the termination or relocation and be provided an opportunity to transfer or receive their records.
