“Patient records under the Department of Health regulations”
Orthotists and prosthetists must maintain patient records which must include:
- The patient name;
- The location and dates of all treatment, evaluation, or consultation;
- The identity of each provider who treated the patient;
- The name of the prescribing physician;
- All prescriptions relevant to the patient; and
- A treatment or service plan.
Orthotists and prosthetists may charge a fee for providing patients with copies of records, which must be $1 per page for the first 25 pages and $0.50 per page for every page after 25 pages. In addition, the actual cost of postage may be added.
