Fees for providing copies of medical records – Ohio Rev. Code Ann. § 3701.741

When a patient or a patient's personal representative requests a copy of the patient's medical record, the health care provider or medical records company that receives the request may charge the following for such copies:
- For data recorded on paper or electronically, $2.74/page for the first 10 pages, $0.57/page for pages 11-50, and $0.23/page for pages 51 and beyond;
- For data from an x-ray, MRI, or CAT scan recorded on paper or film, $1.87/page; and
- For any related postage, the actual cost incurred by the provider or records company.
When a person or entity other than the patient or his personal representative requests a copy of a patient's medical record, the provider or records company that receives the request may charge the following for such copies:
- An initial fee of $16.84, to compensate for the records search;
- For data recorded on paper or electronically, $1.11/page for the first 10 pages, $0.57/page for pages 11-50, and $0.23/page for pages 51 and beyond;
- For data from an x-ray, MRI, or CAT scan recorded on paper or film, $1.87/page; and
- For any related postage, the actual cost incurred by the provider or records company.
On request, a health care provider or medical records company must provide one copy of the patient’s medical record and one copy of any records regarding treatment performed since the original request was made, without charge, to the following entities:
- The bureau of workers’ compensation;
- The industrial commission;
- The department of job and family services or a county department of job and family services;
- The attorney general; and
- A patient, his personal representative, or authorized person if the medical record is necessary to support a claim under Title II or Title XVI of the Social Security Act.
