The health insurance commissioner and secretary of the executive officer of human services must submit an evaluation of the patient-centered medical home model (PCMH) following the second and fourth year of implementation. The evaluation must address: (1) the number of PCMH enrollees; (2) the number of PCHM providers, their geographic distribution, and “the number of primary care physicians per thousand populations;” (3) PCMH performance and quality; (4) the estimated impact of PCHMs on preventive care access; (5) PCMH payment arrangements and the costs related to implementation and the provision of care coordination payments; (6) “the estimated impact of [PCMHs] on health status and health disparities;” and (7) “estimated savings from [PCMH] implementation.”

Health insurers must submit utilization, quality, and financial information regarding PCMH implementation and impact to the secretary.
