Establishment and Requirements of Chronic illness Demonstration Projects – N.Y. Soc. Serv. Law § 364-l
The commissioner of health may develop, subject to funding, demonstration projects to (1) improve the coordination of care delivered to Medicaid beneficiaries with co-morbidities who are ineligible for managed care; (2) ensure that such persons receive care in the “most appropriate setting; and (3) improve the health outcomes of such persons while reducing the cost of their care.
The commissioner may use these demonstration projects to test various care and reimbursement models (e.g. shared savings). The commissioner report the efficacy of these demonstration projects and make recommendations to the Governor by January 1, 2010.
This statute will expire on March 31, 2013.
