Eligibility and approval
Permits licensed practitioners and pharmacists to enter into collaborative drug therapy agreements. Requires pharmacists to communicate provided care to the practitioner and to document care in the patient’s medical record. Requires the practitioner to also have access to the patient’s medical record. Requires the board of pharmacy and the board of medicine or board of nursing to approve the agreement before it may become effective. Change in the participating practitioners or pharmacists does not necessitate board approval, but must be communicated to all relevant boards. Permits termination of the agreement at will by any party. Prohibits parties to the agreement from participating in an incentive-based program that encourages certain products as a means of obtaining personal financial benefit.
