Hospitals, Physicians, Physician Extenders, and Chiropractors must report to the Department of Health and Human Services any case of an occupational disease no later than 30 days from the date of diagnosis or from discharge from a hospital. The report should include any factor known to the physician which is suspected of being a contributing factor to the disease, including whether or not the person smokes and, if so, how often. Additionally, physicians must report such identifying information as sex, birthdate, race, occupation, industry, employer name, name of disease, etc. If notified by the department, a physician must report any further information requested concerning any person now or formerly under his care, diagnosed as having an occupational disease.
