Discharge Planning
Requires an interdisciplinary team to develop a discharge plan at least 30 days before discharging a recipient of ICF/MR services. Requires the discharge plan to include interviews between the recipient and the referral agency, a recipient tour of their prospective placement, and counseling of the patient and their family regarding the transfer to the new facility.
