Maine defines a sentinel event as the following:
- An unanticipated patient death or transfer to another health care facility unrelated to the patient’s underlying condition;
- A major permanent loss of function unrelated to the patient’s natural course of his or her underlying condition;
- An unanticipated perinatal death or permanent loss of function in an infant unrelated to the natural course of the infant or mother’s illness or underlying condition;
- Other serious and preventable events identified by a nationally recognized quality forum.
