The law states that it is Medicaid fraud, a Class D felony, for any person to:
1. Knowingly or intentionally files a false Medicaid claim;
2. Obtains payment from the Medicaid program by false or misleading information;
3. Acquires a Medicaid provider number by unauthorized means;
4. Alters documents with the intent to defraud the Medicaid program;
5. Conceals information in order to receive unauthorized Medicaid payments.
