Information contained in the “information for medical and health use only” section of a birth record may be disclosed in accordance with the following:

As part of a certified copy of the birth record if specifically requested by:
- The individual to whose birth the record attests;
- The individual’s parent(s), guardian(s), or lineal descendant(s); or
- A federal or state government or political subdivision official involved in law enforcement, investigation or criminal prosecution.
- By the office of vital statistics or a local registrar if ordered by a court; and
- By the office of vital statistics or a local registrar if the state registrar specifically authorizes release of the information for statistical or research purposes under conditions of the state registrar, subject to the approval of the director of health.
