For each individual that receives assisted living services, the program must keep a record that includes identifying information of the consumer and his or her emergency contact, a functional assessment of the consumer, a service plan, an authorization for release of information and documented proof of legal representation. The program must provide the consumer with a functional assessment within 30 days of receiving services, and must create a service plan for the individual that is updated every 6 months. Finally, when acquiring an authorization for release of an individual’s information, the program must provide the consumer an authorization form that is in language the individual can understand, with certain information included as described by the Department of Health and Human Services.
