The department of developmental disabilities will keep in its office, accessible only to its employees or by consent of the department or the order of a judge, a record of every resident in an institution governed by it, including the following information:
- Name;
- Residence;
- Sex;
- Age;
- Nativity;
- Occupation;
- Condition;
- Date of entrance or commitment;
- Date, cause, and terms of discharge;
- The condition of the patient at the time of leaving the institution;
- All transfers from one institution to another; and
- The date and cause of death, if such person dies while in the care or custody of the department.
The managing officer of an institution must report to the department any accident, injury, or peculiar death of a resident within 24 hours, giving the circumstances as fully as possible.
