The department of human services must establish a “long-term care options counseling program” that will provide individuals with information regarding their options for obtaining and paying for long-term care services. Long-term care counseling must be made available to all individuals that are admitted or seek admission to a long-term care facility and must be provided to persons that apply for Medicaid long-term care services.
The Assessment and Coordination Unit (ACU) will assess applicant’s medical, social, and environmental needs to determine their eligibility to receive home and community based service, medication management, and other federally authorized long-term care services. This assessment will subsequently form the basis of a care plan for each applicant.
