Abortion and miscarriage data - Me. Rev. Stat. tit. 22, § 1596

For every abortion performed in Maine, a report must be made to the Department of Health and Human Services on forms prescribed by the department.  These forms may not identify the patient by name, and may contain only the information requested on the United States Standard Report of Induced Termination of Pregnancy, published by the National Center for Health Statistics.  The form must be prepared and signed by the attending physician and delivered to the department not later than 10 days following the end of the month in which the abortion is performed.  A physician who reports data on an abortion shall be immune from any criminal liability for that abortion.

For each miscarriage that occurs, a report must be made by the physician in attendance on forms prescribed by the Department of Health and Human Services.  These forms must contain all of the applicable information required on the certificate of fetal death in current use.  The attending physician must prepare and sign the report, and transmit it to the department not later than 10 days following the end of the month in which the miscarriage occurs.  The identity of any patient or reporting physician is confidential and the department shall take the steps which are necessary to ensure that confidentiality
