“Medical Records and Information”
This section establishes how carriers shall make good faith efforts to obtain records relevant to the review of an insured’s claim and not in the possession of the carrier from providers. It is establishes protocol for both non-expedite reviews and expedited reviews. “Good faith efforts” shall be defined as no fewer than three documented attempts by the carrier to obtain the information from a provider or the provider's designee.
