“Disclosure of information (general) under the secretary of state regulations”
Every licensed clinical mental health counselor, marriage and family therapist, or psychotherapist must provide a disclosure statement to a client no later than the third office visit. The disclosure statement must be signed by the client. The disclosure statement may be given to the client’s parent or guardian if the client is a minor or incapable of understanding the disclosure statement.
