Health care providers must report diseases that are dangerous to public health to the Department of Health and Social Services’ Division of Public Health. The reports must contain the following information: a patient’s contact information, date of birth, race, ethnicity, gender, and the patient’s medical information. Patient information may be disclosed to the Division of Public Health without a patient’s authorization.
