“Authority and obligations of health care providers, health care facilities and residential care facilities and health insurers under the Agency of Human Services regulations”
Health care providers, health care facilities, and residential care facilities must:
- Obtain and follow a patient’s health care instructions;
- Communicate with the patient, agent, guardian or other persons identified by the patient;
- Recording the basis for all significant decisions in the patient’s medical record, including the basis for believing a patient wants to suspend or revoke a DNR order or identification;
- Assist the patient to execute an advance directive; and
- Assist the patient, agent or guardian and obtaining care or submitting documents to the Vermont Advance Directive Registry.
