“Clinician orders for life sustaining treatment (COLST) under the Agency of Human Services regulations”
At any time a patient may need life sustaining treatment, the patient’s clinician must determine the wishes of the patient regarding life sustaining treatment and record those wishes in the patient’s medical chart. A clinician order for life sustaining treatment (COLST) is a form which is available for health care providers and facilities to summarize orders regarding life sustaining treatment so that the orders are readily accessible to staff who will implement them.
