105. Mass. Code Regs. 130.331. - Serious Incident and Accident Reports

All hospitals must report the following to the Department of Health:

  1. Unanticipated death, unrelated to the patient’s condition, or death as a result of an error;
  2. Evacuation;
  3. Fire;
  4. Suicide;
  5. Serious criminal acts;
  6. Labor strike;
  7. Other serious incidents or accidents.

Hospitals are also required to report to the Department any instances of patient abuse, neglect or mistreatment.  Additionally, hospitals must report other serious reportable events, as defined by 105 MA ADC 130.332, that affects the health or safety of patients, within seven days of the incident.
