Requirements for Hospital Patient Information Reports to the State Department of Health – Ind. Code Ann.§ 16-21-6-6

Each hospital must file with the State Department of Health, inpatient and outpatient discharge data reports that include the following information on the patient:
- Length of stay
- Diagnosis
- Date of admission and discharge
- Type of admission and admission source
- Date of birth
- Gender
- Race
- Type of payor
- Total charge of stay
- Zip code of residence
- Starting October 1, 2013, all diagnosed external causes of injury codes.
