The Center for Medicare and Medicaid Innovation (CMMI) announced the Comprehensive Primary Care Initiative as a way to assist primary care providers in delivering higher quality, patient-centered, coordinated care. CMMI selected 500 primary care practices to participate from across the country. These practices will be paid an incentive payment per patient, and will allow the primary care practices to manage health care needs of patients with multiple chronic conditions, ensure access to care, provide preventative services, and coordinate care across providers. This initiative relies significantly on the use of health information technology to store and share health data across providers.
